S.No. BRIEF
01. The doctors in the OPDs as well as PWOs shall identify the eligible non-critical EWS patients willing/requiring referral to identified private hospitals in the OPDs/OPD registration area.
02. Such identified non-critical patients who are willing to be referred to private hospital can also be directly referred by the PWO (pre-registered) after filling the referral proforma (1+2 copies) and Undertaking. The copy of referral proforma and the Undertaking to be filled by referring doctors are available on LAN of our HIMS.
03. Eligible patients requiring referral identified by various Departments in their OPDs may also be sent to the Help Desk where the PWO shall fill the referral proforma (1+2 copies) and assist the patient/attendant in filling the Undertaking. The said proforma is to be signed by the referring doctor and the Nodal Officer.
04. An e-mail has to be sent to the identified private hospital where the concerned patient is being referred to mentioning therein the following details :-
  • Name of the patient
  • Age
  • Sex
  • CR No., if any
  • Department/diagnosis
  • Hospital referred to
  • Critical/non-critical
