| 01. | The doctors in the OPDs as well as PWOs shall identify the eligible non-critical EWS patients willing/requiring referral to identified private hospitals in the OPDs/OPD registration area. |
| 02. | Such identified non-critical patients who are willing to be referred to private hospital can also be directly referred by the PWO (pre-registered) after filling the referral proforma (1+2 copies) and Undertaking. The copy of referral proforma and the Undertaking to be filled by referring doctors are available on LAN of our HIMS. |
| 03. | Eligible patients requiring referral identified by various Departments in their OPDs may also be sent to the Help Desk where the PWO shall fill the referral proforma (1+2 copies) and assist the patient/attendant in filling the Undertaking. The said proforma is to be signed by the referring doctor and the Nodal Officer. |
| 04. | An e-mail has to be sent to the identified private hospital where the concerned patient is being referred to mentioning therein the following details :-
